Request Medical Records

Authorizing the release of your medical information

At Los Alamitos Medical Center, we keep your healthcare information private. To receive a copy of your medical records, you may either log in to our online Patient Portal or print and fill out the appropriate Authorization to Use and Disclose Health Information. Bring the completed form with your photo ID in order to pick up your records. If someone else will be picking them up for you, that person will need their photo ID and a signed authorization letter from you.
